Linkages depends on the ongoing ability of the Child Welfare and CalWORKs caseworkers to coordinate their efforts. Some key factors in effective coordination of Linkages cases are:

  • Leverage the use of the Linkages joint case management to better engage families in their case goals.
  • Service coordination activities driven by needs, eligibility and resource array.
  • Clarify case management roles and responsibilities of both Welfare-to-Work and Child Welfare case workers.
  • Agree on frequency and circumstances that trigger exchanging information over the course of the case.
  • Document the coordinated services plan to confirm agreement among the family and team about how the case goals will be met.

Most counties use a linked case plan model to coordinate services. Separate case plans are maintained for WtW and CWS, but there is an additional brief document that summarizes, streamlines and overlays the plans underneath. This helps coordinate the family’s services and activities to meet their safety and economic self-sufficiency goals.
