Evidence-Based Practice Data Collection & Reporting
The following resources and templates support data collection and reporting to help monitor Evidence-Based Practice (EBP) implementation in California. Access the Executive Summary to learn more about the framework for measuring EBPs within FFPS.
Fidelity and Outcomes Reports and Templates
FFPS fidelity and outcomes data collection is supported by two resources: required CWS-CARES reports for submitting aggregate data and optional templates for tracking individual-level data and preparing report totals.
Required CWS-CARES Fidelity and Outcomes Reports only contain aggregated data points and are required for all counties and Tribes participating in the FFPS Program. Counties will be able to view data as Tableau dashboards in CWS-CARES.
Optional Fidelity and Outcomes Templates can be used by counties to track individual-level Fidelity and Outcomes data. The aggregate tabs can also be used to generate the data points needed for the Required CWS-CARES Fidelity and Outcomes Reports. The templates are optional because providers might have access to comprehensive databases maintained by the EBP model developer/purveyor or use their own systems for data capture instead of using these templates.
